Australia's Smith off field after being struck on helmet
Australia's Steve Smith was struck on the helmet while batting and did not take the field at the start of South Africa's second innings on the fourth day of the first Test at Kingsmead on Monday.
Smith ducked into a bouncer from fast bowler Anrich Nortje and received a lengthy on-field assessment before continuing to bat.
He took his score from two to 27 before being caught on the boundary off off-spinner Simon Harmer.
Australia hurried to 314 for six in 58 overs before declaring, setting South Africa 421 to win.
An Australian team spokesman said Smith was "undergoing further assessment" and an update would be provided either later on Monday or on Tuesday.
If Smith is diagnosed with concussion he could be a doubt for the second Test starting in Gqeberha on Sunday.
